Identifying GE power amp

I have found a very nice mono GE power amp that I am trying to identify and repair. It has four 6V6GT tetrodes, a 6SN7GT dual triode and a 5U4G rectifier. The case has the number 4412 stamped into it. Any help would be appreciated. Specifically, I'm looking for a schematic.

Yes Carl, that is the one! Thank you for your help. I do have a question for you or someone who knows about these. There is a pot for biasing the heater voltage depicted to the right of the transformer on the low voltage circuit. This type of bias is foreign to me. What is the proper way to make this adjustment?

Also, the build quality on this amp is just remarkable. I expect that it will sound very nice once I recap it. What's the chance of finding another; are they common?

I believe that is a hum control. Some older tube sets had that and you adjust it for minimum hum
