Closest I could find was 660. It has 3 #45's, 5Z3, 30 and 1B5. 1B5 is a triode like 30 with extra diodes. It may be a similar amp?

You may be able to purchase a repair manual from Always Jukin, they are on the net.
I have a Wulitzer model 616A from 1937 I have had it for about 15 years and it plays perfect, It's built like a tank.
The amp in mine is a number 771, it has two #30 a tube I can't read and four #45s. Looks like yours might be older than 1937, but not much. I think they started making them around 1934 under the name of Wulitzer Simplex.
