composite ceramic capacitor

The Halicrafter 5R10 radio I thought I was finished with lost a composite ceramic capacitor, (literally fell apart), and I'm having trouble finding a replacement. Before it fell off its mount their was no hum or distortion, just smooth operating and clear signals.
The capacitor includes a 5000pf, 2x220pf, and 2000pf capacitance at 500volts. It looks like a tubular resistor. Any help finding a replacement would be greatly appreciated.

If you are talking about C12, those all seem to be standard values, and none of them are critical....would you be willing to replace them with individual caps?

:I had a brain fart. The Packaged Electronic Circuit or PEC is a thin flat brown plastic combination of four capacitors with 4 wires coming off of it. One leg is Tied to the tube that shares the 220K 1/2 watt resistor that looks like a tube with metal pressure plates on each end. I thought that was the capacitor. I replaced one that looked like it in another radio. I replaced the resistor with a carbon type and turned the radio on. The clarity of sound improved about 50 percent, and it was already good. On SW I hear no hum, just stations as I troll the dial. Amazing.
