That tube complement does not compute. Basically it is six audio output tubes, three triodes, and a rectifier.
The 6K6 seems bogus here.

::i cleaned a little better and you are right,the tubes are4-6k7,3-76,2-45,and 1 80

20-CM-576 might be close, in the Resources section under Gamble-Skogmo. It has a similar tube type complement EXCEPT for the output tubes, which are 6F6s. The use of 45s seems a little anachronistic in a set using 6K7 tubes, but maybe yours was a "transition" model.
